Dr. Jacqueline Zimmerman Departure Notice

Jacqueline Zimmerman, DO, will no longer be practicing with Hendrick Clinic Obstetrics and Gynecology and will be transitioning to private practice under the name EmpowHer Women's Health & Wellness, effective October 30, 2023.

If you choose to continue your care with her in this new practice, a medical record release is not required at this time. However, if additional information is needed, you may be asked to fill out a release form at a later date.

Dr. Zimmerman's office will be located at 1326 Pine Street, and the phone number will be 325-261-8883. The upcoming website domain will be empowherwomenshealth.com.

If you would like to continue your care within Hendrick Clinic, you may request to have your next appointment with any of the other Hendrick Clinic providers listed at hendrickclinic.com. If you stay within Hendrick Clinic, you will not need to request your records.

If you choose to seek care outside of Dr. Zimmerman or Hendrick Clinic, please fill out the medical records request form at 1924 Pine Street, Suite 401A.
